Offering Hope with a Prostate Cancer Diagnosis

Presentation

An employee based in France reached out to PHM for timely assistance regarding his 66-year-old father’s newly recharacterized prostate cancer.

A recent rise in PSA levels prompted prostate biopsies, which indicated low-grade, low-risk prostate cancer.

However, suboptimal imaging completed at a remote location in France suggested locally advanced disease requiring immediate treatment, conflicting with the original biopsy findings. Given that this information could significantly impact the treatment plan, the family was distressed when they contacted us, seeking additional guidance and education.

PHM Actions

A multidisciplinary team (including clinical, logistical, and research members) was swiftly assembled to address the client’s needs.

The care team worked to quickly get historical medical records translated.  

Recognizing the urgency of engagement due to an upcoming consultation with a Paris based expert, the team immediately jumped in by delivering a question list, to optimize the critical physician consultation.  

In addition to providing a question list, family members had immediate access to a PHM Oncology Clinician, to ensure comprehensive understanding of the goals of this visit.  

As prompted by the question list, the Paris based expert agreed with more advanced imaging recommendations, specifically a PSMA PET scan.  

Research team created and presented a comprehensive “Treatment Option Plan”, reviewing prostate cancer diagnosis, treatment and prognosis, allowing the patient to assume more autonomy in his care.  

Provided reassurance that we will continue to support later this year at the time of confirmatory biopsy testing.  

Outcome

Immediate reassurance on welcome call that low grade, low volume localized prostate cancer does not often present with bilateral lymph node involvement reducing patient anxiety and reinforcing decision of second opinion.

Tailored consultation question lists ensured optimal consult and the appropriate diagnostic work-up e.g., PSMA PET  

Real-time education and support during evolving diagnosis, including compelling data for active surveillance, facilitated informed decision making and reassurance.
